The Gut Health – Lipoedema Link: What the Research Says

Lipoedema is a chronic and often misunderstood condition that primarily affects women. It’s characterised by an abnormal, symmetrical build-up of painful fat and fluid, usually in the legs, arms and hips. which doesn’t respond to typical weight loss methods like diet and exercise. While the exact cause of lipoedema remains unclear, emerging research is beginning to highlight the possible role that gut health plays in its development and progression.

As practitioners, we’ve long understood the link between the gut and systemic inflammation, hormone balance, and fat storage—all of which are implicated in lipoedema. Let’s looks at what the science says and how supporting the gut may offer relief, prevent disease progression and provide better long-term outcomes.


Gut Health: A Hidden Driver of Inflammation and Hormone Imbalance

The gut is home to billions of bacteria that make up the microbiome. These microbes help regulate digestion, metabolism, immune function, and even hormone levels. When the microbiome is disrupted—a condition called dysbiosis—it can lead to increased inflammation, poor detoxification, hormonal imbalances, and metabolic dysfunction.

Lipeodema is now increasingly recognised as an inflammatory condition, and this is where gut health becomes especially important.


What the Research Says

1. Inflammation and lipoedema

Research has found that people with lipeodema have elevated levels of inflammatory markers such as TNF-α and IL-6 [1]. Chronic inflammation can alter fat metabolism, worsen tissue fibrosis, and increase pain sensitivity. Since the gut microbiota is a key regulator of inflammation, an unhealthy gut may worsen or even trigger lipoedema symptoms.

2. Leaky Gut and Systemic Inflammation

A compromised gut lining—often referred to as “leaky gut” or intestinal permeability —can allow bacterial toxins like lipopolysaccharide (LPS) into the bloodstream, triggering systemic inflammation. A 2021 study published in Frontiers in Immunology discussed how gut-derived LPS contributes to “metabolic endotoxaemia”, which may play a role in fat deposition and metabolic dysfunction [2]. These mechanisms may help explain the weight-independent fat accumulation seen in lipoedema.

3. The Estrobolome and Hormonal Triggers

Lipoedema often begins or worsens at times of hormonal change—puberty, pregnancy, or menopause—suggesting a strong hormonal component. The gut microbiome plays a crucial role in metabolising and detoxifying oestrogens through a group of bacteria called the estrobolome. An imbalanced estrobolome can contribute to excess circulating oestrogens, promoting fat deposition and fluid retention.

4. Microbiome Diversity and Fat Storage

Certain microbiome imbalances have been linked to weight-independent fat gain. While much of this research has focused on obesity, there are parallels with lipeodema. For example, low microbial diversity and an overgrowth of pro-inflammatory bacteria may be associated with metabolic issues and disproportionate fat accumulation [3].
