Understanding Hormones in Menopause: Navigating the Transition with Clarity

The transition into menopause, also known as perimenopause is a natural but often challenging phase in a woman’s life. It typically begins in the  40s to early 50s and can last for several years before menopause is reached (defined as 12 consecutive months without a menstrual period). This transition is governed by a complex interplay of hormones that influence everything from your cycle and sleep to mood, metabolism, and libido.  You might have heard of some of the hormones, but do you understand what they do, and how the interact? 

Understanding how these hormones shift and how stress and lifestyle factors can impact them can empower you to navigate this life stage with greater ease, confidence, and well-being. 

Key Hormones in the Menopause Transition 

Oestrogen 

Oestrogen is central to a woman’s reproductive system, but it also affects brain function, cardiovascular health, skin, bones, and mood. It supports tissue elasticity, collagen production, and plays a role in regulating body temperature. 

As women approach menopause, oestrogen levels begin to decline but not in a straight line. They often fluctuate erratically, which can trigger symptoms like hot flushes, night sweats, mood changes, brain fog, and vaginal dryness. Over time, oestrogen levels settle at a lower baseline, contributing to long-term risks such as osteoporosis and cardiovascular disease. 

Progesterone 

Progesterone is produced mainly after ovulation and helps regulate the menstrual cycle, prepare the uterus for pregnancy, and calm the nervous system. It balances oestrogen and has sedative, mood-stabilising effects. 

In perimenopause, ovulation becomes irregular, leading to a more abrupt decline in progesterone than oestrogen. This imbalance can cause symptoms such as irregular or heavy periods, anxiety, disrupted sleep, and PMS-like mood swings. 

Testosterone 

Often overlooked in women’s health, testosterone plays a vital role in libido, motivation, muscle maintenance, and cognitive sharpness. Women produce testosterone in the ovaries and adrenal glands. 

As women age, testosterone levels gradually decline. Low levels may contribute to reduced sex drive, lower energy, and decreased muscle tone. 

DHEA 

DHEA (dehydroepiandrosterone) is a steroid hormone made in the adrenal glands. It acts as a precursor to oestrogen and testosterone and supports resilience, immunity, energy, and mood. 

DHEA levels naturally decline with age, but they can fall more sharply in those under chronic stress. This can reduce the body’s ability to make the hormones it needs post-menopause, contributing to fatigue, low libido, and reduced vitality. 

Cortisol 

Cortisol is the primary stress hormone, essential for survival. It helps regulate blood sugar, inflammation, immune responses, and energy levels. However, chronic elevation can cause problems. 

In perimenopause, cortisol levels often become dysregulated rising too high at night (disrupting sleep) or staying persistently elevated throughout the day. This can worsen fatigue, increase central weight gain, and intensify anxiety or mood swings. 

The Impact of Stress on Hormonal Balance 

Stress is often the missing piece in the hormonal puzzle. When the body perceives ongoing stress whether emotional, physical, or environmental—it shifts into a state of self-preservation. This survival mode has widespread effects on the endocrine system: 

  • Progesterone levels drop: Under stress, the body redirects hormone production away from progesterone to prioritise cortisol a concept known as “pregnenolone steal.” This contributes to anxiety, insomnia, and cycle irregularity. 
  • Oestrogen metabolism is disrupted: High cortisol levels can interfere with the detoxification and recycling of oestrogen in the liver, leading to symptoms of both deficiency and excess. 
  • DHEA production declines: Since DHEA and cortisol share a common precursor, prolonged stress pushes the body to produce more cortisol at the expense of DHEA. Over time, this reduces resilience, libido, and energy. 
  • Testosterone is suppressed: Cortisol can blunt the body’s ability to produce or convert testosterone, affecting drive, mood, and muscle tone. 
  • Sleep and blood sugar are affected: Cortisol imbalance disrupts circadian rhythms and blood sugar regulation, contributing to fatigue, cravings, and inflammation. 

Effectively managing stress isn’t just about feeling calm it’s essential to restoring hormonal balance. Nervous system regulation, adrenal support, rest, and recovery are foundational pillars of menopause support. 

Symptoms Rooted in Hormonal Imbalance 

The hormonal shifts during menopause give rise to a wide variety of symptoms, such as: 

  • Hot flushes and night sweats: Triggered by fluctuating oestrogen 
  • Mood changes and anxiety: Linked to low progesterone and cortisol dysregulation 
  • Sleep disturbances: Often due to low progesterone and elevated night-time cortisol 
  • Fatigue and low libido: Associated with low testosterone, DHEA, and adrenal overload 
  • Weight gain: Particularly around the middle, often influenced by cortisol and declining oestrogen 
  • Brain fog and memory issues: Related to declining oestrogen and DHEA 

Understanding the hormonal cause of these symptoms helps guide more targeted and effective support strategies. 

Hormone Testing: When and Why 

Menopause is often diagnosed based on symptoms and cycle changes, rather than bloodwork alone. However, testing can be helpful in some cases—particularly when symptoms appear early, are difficult to interpret, or when underlying hormonal patterns need to be clarified. 

Depending on individual needs, testing may explore: 

  • The relationship between oestrogen and progesterone 
  • Cortisol rhythm and adrenal function 
  • Androgens such as testosterone and DHEA 
  • Pituitary signals such as FSH or LH 

Because hormone levels fluctuate especially in perimenopause test results are most useful when interpreted alongside your symptom picture and health history.
